Essential Functions and Responsibilities
The successful candidate will be responsible for reviewing clinical records, monitoring the quality and accuracy of documentation, supporting compliance, and identifying areas where improvements or additional staff training may be required.
1. Clinical Record Reviews (CRR)
Conduct regular Clinical Record Reviews (CRR) to ensure that patient records are complete, accurate, and consistent with required standards.
Reviews should focus on:
Verifying that all required clinical documentation is complete and accurate.
Reviewing the quality of care provided to patients.
Checking visit utilization and ensuring that visits are properly documented.
Confirming that care is being provided according to the approved plan of care.
Ensuring staff follow agency policies and procedures.
Reviewing documentation to confirm communication between all members of the care team and other relevant disciplines.
Identifying missing, incomplete, or inconsistent information in patient records.
Ensuring documentation supports appropriate reimbursement and accurate billing.
Identifying potential compliance issues and bringing them to the attention of the appropriate personnel.
The Clinical Record Review process should help ensure that patient care is properly documented while supporting the agency's quality, compliance, and reimbursement objectives.
2. Maintain CRR Compliance
The successful candidate will be expected to maintain the agency's Clinical Record Review compliance rate at 80% or higher.
This will involve:
Conducting reviews consistently and according to established procedures.
Tracking CRR results and identifying recurring documentation issues.
Following up on identified deficiencies.
Monitoring improvements over time.
Reporting significant concerns to the appropriate clinical or administrative leadership.
Supporting corrective actions where compliance falls below the required level.
3. Quality Assurance and Performance Monitoring
Review quality assurance data to identify trends, recurring problems, and areas that may require improvement.
The successful candidate will:
Analyze findings from clinical record reviews and other quality assurance activities.
Identify patterns or trends that may affect patient care, documentation, compliance, or reimbursement.
Communicate significant findings to the Clinical Manager and/or Administrator.
Recommend improvements when recurring issues are identified.
Help determine whether changes to existing processes are necessary.
Identify situations where additional staff education or training may be required.
Assist management in developing appropriate responses to quality and compliance concerns.
The candidate should be comfortable using QA findings to support practical improvements within the agency.
4. Staff Training and Orientation
Participate in agency orientation and in-service training programs as required.
Responsibilities may include:
Supporting the orientation of new employees.
Participating in staff education and training sessions.
Providing guidance on documentation and quality requirements.
Helping staff understand agency policies and procedures.
Supporting training related to identified quality or compliance issues.
Sharing information about common documentation deficiencies and ways to prevent them.
The successful candidate should be willing to contribute to a culture of continuous learning and improvement.
5. Corrective Actions and Process Improvement
The candidate should be able to identify problems and assist in developing practical solutions.
This includes:
Identifying inconsistencies or deficiencies during record reviews.
Determining the potential cause of the issue.
Communicating the concern to the appropriate individual or department.
Developing or contributing to an appropriate action plan.
Following up to determine whether the issue has been resolved.
Recommending changes to processes when necessary.
Monitoring future records to determine whether improvements have been achieved.
The ability to turn quality assurance findings into clear and practical actions is an important part of this position.
6. Compliance and Regulatory Responsibilities
The successful candidate should understand the regulatory requirements that apply to home health services.
This includes knowledge of:
State home health regulations and requirements.
Homecare regulatory standards.
CMS guidelines.
Accreditation standards.
Clinical documentation requirements.
Quality assurance and compliance expectations.
Requirements related to home health records and reimbursement documentation.
The candidate should remain aware of applicable requirements and help ensure that agency practices and documentation remain compliant.
7. Additional Responsibilities
The successful candidate may be required to perform other duties related to quality assurance, clinical documentation, compliance, and agency operations.
Additional responsibilities may be assigned based on the needs of the agency and the direction of the Clinical Manager, Administrator, or other appropriate leadership.
Qualifications and Experience
Candidates should meet the following qualifications and experience requirements:
1. Professional Qualification
Registered Nurse (RN) qualification is preferred.
A current and valid state nursing license is required for candidates applying as an RN.
Candidates should have the professional knowledge necessary to review clinical documentation and evaluate the quality of home health services.
2. Home Health Experience
Applicants should have:
At least 2 years of home health experience.
At least 1 year of experience in quality assurance (QA) within a home health agency.
Practical knowledge of home health operations and clinical documentation.
Experience reviewing patient records and evaluating compliance will be highly beneficial.
3. Regulatory Knowledge
The ideal candidate should have a working knowledge of relevant:
State regulations and requirements.
Homecare regulations.
CMS guidelines.
Accreditation standards.
Clinical documentation requirements.
Home health compliance standards.
A strong understanding of these requirements will help the candidate identify documentation problems and compliance risks during clinical record reviews.
4. Quality Assurance and Audit Experience
Experience in the following areas is preferred:
Clinical chart audits.
Quality improvement programs.
Compliance reviews.
Clinical record reviews.

Monitoring quality improvement activities.
Candidates should be able to review records objectively and identify areas that require correction or improvement.
5. OASIS and EMR Knowledge
Familiarity with OASIS (Outcome and Assessment Information Set) is highly desirable.
Candidates should also have experience or familiarity with:
Electronic Medical Records (EMR) systems.
Home health documentation.
Home health billing documentation.
Clinical chart review processes.
Documentation used to support reimbursement.
Experience working with electronic patient records will be an advantage.
